29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. On or about March 21, 2016 the employer did not ensure that extension cord(s) and related equipment were inspected before each use and removed from service when deficiencies such as, but not limited to, missing ground pins and damaged insulation (sheath) were observed. This condition exposed employees to hazards associated with electrical shock.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: On or about March 21, 2016 the employer did not ensure that scaffolding was enclosed on all open sides. Scaffolding was missing railing at each open end. This condition exposed employees to fall hazards above 10 feet.
