29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: On or about March 21, 2016 the employer did not ensure that scaffolding was inspected before each work shift for visible defects by a competent person and tagged or removed from service when unsafe. Scaffolding was missing railing at open end(s), interlocking pins were lacking between scaffold frames, base plates on uneven mud sills, and a bottom frame post was bent. This condition exposed employees to fall hazards above 10 feet.
