29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. a) 2160 MacArthur Road front - on or about March 22, 2016, employees were using tubular welded frame scaffolding to perform stucco work on a commercial façade, but the employer did not provide an approved means of accessing the scaffold. BEStucco, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 971950, citation number 1, item number 2 and was affirmed as a final order on July 14, 2014, with respect to a workplace located at 2443 Lehigh Street, Allentown, PA 18104.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. a) Front façade of building - On or about March 24, 2016, employees were working on multiple sections of tubular welded frame scaffolding, approximately 13 feet above grade, that did not have a guardrail systems installed along all open sides and ends of platforms. BE Stucco, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(4)(i), which was contained in OSHA inspection number 509219, citation number 1, item number 2 and was affirmed as a final order on September 10, 2012, with respect to a workplace located at 3505 Gettysburg Road, Camp Hill, PA.
