Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to crushing hazards:     Dock Area:  On, or about, March 24, 2016, the underhung monorail crane and hoist was not subject to regular inspections and was not maintained in a safe operating condition.  At the time of the inspection, the crane/hoist was not provided with proper markings and labels, the running rope was worn including a broken wire, flattening and birdcaging, a sheave appeared to be worn and the upper limit switch was either set too high or was not working.
