29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: Ammonia Mechanical Room: On, or about, March 26, 2016, an eye and body wash unit was not provided just outside of the ammonia refrigeration mechanical room where anhydrous ammonia, a corrosive gas, was used as a refrigerant. The only eye and body was unit was located in the room, where it would not have been either unusable or would have created a greater hazard to employees in the event of an ammonia leak.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition: Ammonia Refrigeration Machinery Room: On, or about March 29, 2016, boxes of supplies, power tools and other materials unrelated to the maintenance of the ammonia refrigeration system were stored in the machinery room. Many of the items were combustible and several limited the working space around panels and equipment related to the refrigeration system.
