29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) At the job site, Lot 21 on or about March 29, 2016, several employees who were performing framing work on a new two-story residence were exposed to fall hazards up to approximately 9 feet to concrete deck inside the building and 11 feet onto earthen ground because the workers were not fall protected.      b)  At the job site, Lot 21 on or about March 29, 2016, an employee who was performing framing work from the roof deck was exposed to fall hazards up to 9 feet onto earthen ground because the worker was not fall protected.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    a) At the job site, Lot 21, on or about March 29, 2016, an aluminum extension ladder did not extend 3-feet beyond the 2nd floor joist, exposing employees to an 11-feet fall hazard.      b) At the job site, Lot 21, on or about March 29, 2016, a homemade wooden ladder did not extend 3-feet beyond the 2nd floor deck, exposing employees to an 11-feet fall hazard.

29 CFR 1926.1053(b)(21): Employees were not grasping the ladder with at least one hand when progressing up and/or down the ladder:    a)  At the job site, Lot 21, on or about March 29, 2016, an employee was ascending a 24 foot aluminum extension ladder did not use at least one hand to grasp the ladder, exposing him up to an 16-foot fall hazard.

29 CFR 1926.1060(a)(1): The employer did not provide a training program for each employee using ladders as necessary, enabling each employee to recognize hazards related to ladders and the procedures to be followed to minimize these hazards:     a) At the job site, Lot 21, on or about March 29, 2016, employees using ladders to access the 2nd floor deck and joist areas were not provided ladder safety training, as a result the employees were exposed to a 11-feet fall hazard.
