29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA: a) Job site, 2975 Duss Avenue, Ambridge, PA: On or about March 13, 2016, the employer did not ensure that an in patient hospitalization of an employee following a workplace accident was reported to OSHA within 24 hours. The employee was injured and admitted to the hospital on March 12, 2016, but the incident was not reported to OSHA until March 25, 2016.
