29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by paragraph (l) and shall include the name of the operator, the date of training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation:    On or about, March 29, 2016, each previously trained, evaluated and certified forklift operator was not evaluated prior to the expiration of their certification and the employer did not re-certify employees, documenting the operator's name, date of the training, date of the current evaluation, and the name of the person conducting the evaluation.   Craig Westfall construction, Inc was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard: 29 CFR 1910.178(L)(6) which was contained in OSHA inspection number 1047420,citation number 1,  item number  1b, AND WAS AFFIRMED AS A FINAL ORDER ON 05/18/2015  with respect to a workplace located at 46619 County Highway 2, Griggsville, Illinois, 62340.

29 CFR 1926.451(g)(4)(ii): The top edge height of the top rail on all supported scaffolds was not between 38 inches (0.97 m) and 45 inches (1.2 m) above the platform surface:   On or about March 29, 2016, an employee was exposed to a fall hazard of approximately 11 feet while performing framing operations and the top rail was 32 inches from the platform surface.
