(a) Porte Cochere roof, Provident Village at Creekside located at 4838 South Cobb Drive, Smyrna, GA 30082: The employer failed to provide an adequate fall protection system before his employee began work from the roof at heights of approximately 16-feet, 8-inches from the lower level. No anchor point was provided by the employer for the exposed employee to use as a tie off point. The employer failed to ensure that an adequate fall protection system was being used prior to exiting the site.
