29 CFR 1910.178(m)(4): The employer shall prohibit arms or legs from being placed between the uprights of the mast or outside the running lines of the truck. On March 24, 2016, in the Warehouse, a Crown Reach Truck operator was exposed to a caught in-between hazard by placing his right foot outside the running lines of the truck to prevent the truck from striking a Crown Walkie truck operator who had stopped in front of him.

29 CFR 1910.178(n)(1): All traffic regulations shall be observed, including authorized plant speed limits. A safe distance shall be maintained approximately three truck lengths from the truck ahead, and the truck shall be kept under control at all times. On March 24, 2016, in the Warehouse, X area, a Crown Reach truck operator was exposed to a struck-by hazard by not maintaining a safe distance from the Crown Walkie operator in front of him and struck the Walkie in the rear.
