29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems or personal fall arrest systems: On or about March 24, 2016 and at times prior thereto, employees were working on a residential home installing felt paper. One employee was working on the roof without any form of fall protection. The employee was exposed to fall hazards of greater than six feet to the ground below.

29 CFR 1926.503(a)(1): A training program was not provided to each employee who might be exposed to fall hazards, to enable that employee to recognize the hazards of falling, and be trained in the procedures to be followed to minimize these hazards: On or about March 24, 2016 and at times prior thereto, the employer did not provide a fall protection training program to employees. Employees were exposed to fall hazards of more than six feet.
