29 CFR 1926.501(b)(1): Employees on walking/working surfaces with an unprotected side and/or edge which is 6 feet or more above a lower level were not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: a) Worksite located at 58 North Main St, Castleton-on-Hudson, NY, on or about March 30, 2016, employees of Monolith Solar Associates, LLC were not wearing complete fall protection while installing solar equipment on a roof, 25 feet above the ground.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Worksite - located at 58 North Main St, Castleton-on-Hudson, NY, on or about March 30, 2016, employees of Monolith Solar Associates, LLC, were not trained on fall protection, and were exposed to a 25 foot fall hazard while installing solar equipment.
