29 CFR 1926.451(b)(8): At all points of a scaffold where the platform changes direction, such as turning a corner, the platform that rests on a bearer at an angle other than a right angle was not laid first, and platforms which rest at right angles over. On or about 30 March 2016, at this construction site, employees were not protected from fall hazards when moving from one scaffold section to another ,at a corner, and where no access transition was provided.

On or about 30 March 2016, at this construction site, employees were exposed to fall and struck-by hazards while performing bricklaying operations on a tubular framed scaffold which was erected with base plates that were not fully placed on the mud sills.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. On or about 30 Mar 2016, at this construction site, employees worked from a scaffold platform and used cross braces as a means of access to the work area.
