29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective. a. On or about 03/30/16 in the Air Freshener Area, an employee was exposed to a TPI Industrial brand pedestal fan that did not have a grounding pin. The grounding pin had been broken off from the fan's plug.

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed. a. On or about 03/30/16 in the Storage Area, a fuse box had empty fuse sockets and did not have a front cover, exposing employees to electrical hazards. Employees accessed the fuse box daily to turn on and off the overhead fluorescent lights.
