Colmar, PA. 2nd Level: On or about March 29, 2016, a carpenter was using a Skytrak 8042 Telehandler to raise and horizontally extend a scaffold platform to a working height of approximately 15 feet above grade, but the scaffold platform was not an approved attachment for this model of tele-handler.
