29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:    a) 111 Brentwood Ct. Colmar, PA. 2nd Level: On or about March 29, 2016, a carpenter was using a Skytrak 8042  Telehandler to raise and horizontally extend a scaffold platform to a working height of approximately 15 feet above grade, but the scaffold platform was not an approved attachment for this model of tele-handler.
