29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) For employees working at 2717 Santa Barbara Blvd in Cape Coral, as observed on or about March 30, 2016. Employees were installing roofing underlayment on the low pitch roof. The roof to ground height measured approximately 21 feet.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a) For employees working at 2717 Santa Barbara Blvd in Cape Coral, as observed on or about March 30, 2016. Employees were installing roofing underlayment on the low pitch roof. The employees were using an extension ladder to access the roof. The ladder was fully extended and was still approximately 1.5 feet below the roof edge. The roof to ground height measured approximately 17 feet 5 inches.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall: a) For employees working at 2717 Santa Barbara Blvd in Cape Coral, as observed on or about March 30, 2016. Employees were using an extension ladder to bring material up to the roof. The employee was climbing the ladder while carrying two approximate 28 pound rolls of underlayment over his shoulder. The employee was exposed to an approximately 17 feet 5 inch fall hazard.
