29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about March 29, 2016, the employer did not ensure employees engaged in new residential roofing activities were using fall protection, exposing the employees to a fall from a height greater than six (6) feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  On or about March 29, 2016, the employer did not ensure employees engaged in new residential roofing activities, and working at a height greater than six (6) feet above ground level, had not been provided fall protection training.  Employees were exposed to fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about March 29, 2016, the employer did not ensure the side rails of extension ladders, being used by employees engaged in roofing activities to access the roof, extended at least three (3) feet above the roof's landing.  This exposed the employees to a fall from a height greater than six (6) feet to the ground below.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects or faulty were not immediately tagged "Do not Use" or marked in a manner readily identifying them as defective, and were not withdrawn from service.  On or about March 29, 2016, the employer did not ensure ladders with structural defects were removed from service.  This exposed employees to falls at a height greater than six (6) feet to ground level.
