29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.     At the site; the employer did not ensure that excavated material and equipment were kept at least 2 feet away from the edge of excavations:    a)    On or about March 30, 2016; excavated material was not kept at least 2 feet away from the edge of the excavation, exposing employees to struck-by hazards.    b)    On or about April 4, 2016; a KOMATSU PC490LC Hydraulic Excavator not kept at least 2 feet away from the edge of the excavation, exposing employees to struck-by hazards.
