29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources. Production area - On or about March 24, 2016, sanitation employees were required to perform tasks on ML-2 Mixer including, but not limited to cleaning and sanitizing the Waukesha pumps. The employer did not ensure that the employees were provided locks or other hardware to isolate energy sources prior to performing the tasks. Employees were thereby exposed to laceration / amputation hazards.

Production area - On or about March 24, 2016, sanitation employees were required to perform tasks on the ML-2 Mixer, including but not limited to cleaning and sanitizing the Waukesha pumps. The employer did not ensure that each employee received training as an authorized employee prior to performing the tasks. Employees were thereby exposed to laceration / amputation hazards.
