On or about 2/5/2016 at 1035 Scales Road Suwanee, GA, Building 8; Employee(s) engaged in framing activities of a second floor stairwell landing were exposed to fall hazards of 21-feet to the lower level.  The job-made scaffold being used was not designed by a qualified person.   b. A job-made scaffold, available for employee use below the first floor stairwell landing, was not designed by a qualified person and exposed employees to falls 10 feet to the lower level.

On or about 2/5/2016 at 1035 Scales Road Suwanee, GA, Building 8,employee(s) engaged in framing activities of a second floor stairwell landing were exposed to fall hazards of 21-feet to the lower level.  The job-made scaffold being used was not erected under the supervision and direction of a competent person.   b. On or about 2/5/2016 at 1035 Scales Road Suwanee, GA, Building 8, a job-made scaffold located below the first floor stairwell landing, exposing employees to falls of 10-feet to the lower level, was not erected under the supervision and direction of a competent person.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a. On or about February 5, 2016 at 1035 Scales Road Suwanee, GA, Building 8; an employee performing framing operations from a job-made scaffold was not protected against falling 21-feet to the concrete lower level.    b. On or about February 5, 2016 at 1035 Scales Road Suwanee, GA, Building 8, workers were exposed to falls of 10-feet to the lower level where planks, creating a job-made scaffold, were used as a working surface immediately below the incident related, job-made scaffold.  No fall protection was provided.
