29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) Endurance Roofing, LLC, 2208 Clancy Court, Brighton, CO 80601: On and before March 31, 2016, the controlling and exposing employer did not ensure that employees were wearing safety glasses when using pneumatic nail guns to shingle a residential roof. This condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: a) Endurance Roofing, LLC, 2208 Clancy Court, Brighton, CO 80601: On and before March 31, 2016, the controlling and exposing employer did not ensure that employees performing roofing work on a residential roof were protected from falling. Up to six employees were working on this roof, with up to a 5 on 12 pitch, without the use of personal fall arrest systems. This condition exposed employees to fall hazards of up to approximately 16 feet.
