29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6-feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of '1926.502(k) developed and implemented.    At the new residential construction site, employees installing shingles on the roof without fall protection were exposed to fall hazards of more than 18 feet to the lower level.

29 CFR 1926.1053(b)(1):  When portable ladders were used for access to an upper landing surface, the ladder side rails was not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    At the new residential construction site, employees accessing and egressing the roof on a ladder that was extended less than a foot above the roof edge were exposed to fall hazards of more than 18-feet.
