29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      On or about March 31, 2016, the employer did not establish an energy control program to ensure employees who performed service and/or maintenance including set-up on machines or equipment were protected from unexpected energizing, start up or release of stored energy. Employees performed set-up on vertical lathes, CNC lathes, manual lathes and CNC milling machines are were exposed to electrical, pneumatic, hydraulic, kinetic, etc. energy sources.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about March 31, 2016, employees were exposed to caught-in, caught-by, or struck by hazards when the employer required the employees to operated the follwing machinery without proper guarding.    a. An employee was observed operating a Bridgeport vertical drill press without proper chuck guards.    b. An employee was observed operating a Baldo Reliance belt sander with no machine guarding.     c. An employee was observed operating a Okuma & Howa ACT 20, AHL 20 and ACT 4 CNC lathes with no interlock. An employee was observed opening the doors and the machines kept running.
