29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or personal fall arrest system. a) Rear Top Level Center of Structure - Worker was constructing a concrete wall at the edge, not protected from falling approximately 25 feet to the concrete slab below, on or about 3/31/2016. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel: a) Outside Front Center of Structure - A worker was standing on a stack of 7 wooden pallets, also on the pallets was a large bin. A second worker, forklift operator, used a forklift to lift the unprotected worker, pallets and bin approximately 25 feet to the top level of the steel framed structure, exposing the worker to a fall hazard, on or about 3/31/2016. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
