(b) Evoqua Water Technologies, 2200 Fort Amanda Road, Lima, OH 45802; On or about March 31, 2016, the employer did not ensure that machine specific lock out tag out procedures contained procedural steps that were sufficiently specific to identify the energy sources which should be controlled.
