On March 31, 2016, on the 4th floor, the ladder access hole does not have a guardrail around the opening, exposing employees to a 10 feet fall hazard. b. On March 31, 2016, on the 5th floor, the ladder access hole does not have a guardrail around the opening, exposing employees to a 10 feet fall hazard.
