29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered. Jobsite: On or about 30 March 2016, an employee, called the "bull pusher," working in and around the furnace area on the main foundry floor was improperly wearing his PPE and exposing himself to contact with a hot substance that could cause severe burns. The bull pusher was observed wearing his flame retardant jacket unbuttoned exposing his torso to severe burns from the splashing of molten iron while moving and pouring the molten iron.
