29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Jobsite located at 342 South Richard Street, Bedford, PA 15522: Observed on or about March 31, 2016, employees used a potable extension ladder to access the roof of a multi story building. The ladder side rails did not extend at least 3 feet (0.9 m) above the upper landing surface. Employees using the ladder to access the roof were exposed to a fall hazard of approximately 30 feet to the ground below.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Jobsite, 342 South Richard Street, Bedford, PA 15522: Observed on or about March 31, 2016, employees were engaged in roofing work on a steep roof with a slope of approximately 18 in 12 (vertical to horizontal) while not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. The employees were exposed to a fall hazard of approximately 30 feet to the ground below.
