29 CFR 1926.405(b)(1): Openings in boxes, cabinets, and fittings through which conductors entered were not effectively closed: a) 145 Colorado Street, Chambersburg, Pa - Mechanical room had live branch circuits wired directly to an uncovered electrical pull box where branch circuits were wired outside of the box, thereby exposing employees to an electrical shock on or about March 30, 2016. No abatement certification or documentation required.
