29 CFR 1926.501(b)(10):  Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted:     On or about April 4, 2016, employees were exposed to a fall hazard greater than 6 feet to the ground level below, while installing solar panel equipment on a metal roof without the use of fall protection equipment or an alternative fall protection plan.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about April 4, 2016, employees were exposed to a fall hazard greater than 6 feet to the ground level below, while installing solar panel equipment on the metal roof without being trained to recognize the hazards of falls.

29 CFR 1926.1053(b)(5)(i):  Non-self-supporting ladders were not used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):    On or about April 4, 2016, employees were exposed to a fall hazard of greater than 6 feet, while accessing the roof on an extension ladder placed greater than one-quarter of the working length of the ladder.
