The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a) Quality Exteriors, LLC. site located at 2490 Jackson St. Oshkosh, WI 54901: On April 4, 2016 the employer had not provided fall protection training for employees working at heights greater than six installing exterior wood siding on a new three story apartment building. employee were not trained to recognize fall hazard conditions such as working from unprotected balconies and not using fall protection equipment while operating aerial lift up at third story level.
