29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a. On or about April 4, 2016, on an 8:12 pitch roof, three employees performing re-roofing operations were not protected from an 18-foot 4-inch fall hazard by use of a guardrail system, safety net system or personal fall arrest system.
