29 CFR 1910.67(c)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.  An employee working for Madison Tree Care & Landscaping, Inc. was observed working inside Lift-All articulating/telescopic aerial lift bucket without a body belt/harness and lanyard attached to the boom or basket while exposed to a fall hazard approximately 30 feet above the ground below.
