29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:    a)  Main shop:  On or about March 31, 2016, an employee was exposed to a struck-by hazard, while operating a Clark forklift, Model GCX25, which had an addition installed, a Vestil Boom attachment, Model LM-EBT-4-24, without manufacturer's approval.    b)  Main shop:  On or about March 31, 2016, an employee was exposed to a struck-by hazard, while operating a Clark forklift, Model GCX25, which was modified by installing the forks upside down, without manufacturer's approval.
