29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a.) The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1, Item 1 for the workplace hazard assessment that was not put in writing, certified with the person conducting the evaluation, and the dates of the assessment.  b.) The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1, Item 2 for not have running water in the facility for drinking, washing or personal service rooms.   c.) The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1, Item 3a for not developing a written hazard communication program.   d.) The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1, Item 3b for not training employees on the hazard communication standard which encompassed the Global Harmonization System (GHS).

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:   On or about April 5, 2016, the workplace hazard assessment had not been put in writing and certified with the person conducting the evaluation and the dates the hazard assessment was completed. Personal protective equipment required to be worn included safety glasses and chemical resistant gloves.

Employees are working with hazardous chemicals such as coolant, gasoline, and motor oil.
