29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: Anatomic Dept.: On or about April 7, 2016, the employer did not ensure protective equipment such as, but not limited to face shield, respirator, lab coat, apron and safety glasses were not being worn where necessary to protect against splash hazards. 29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment: Histology Utility Room: On or about April 7, 2016, where employees were exposed to potential splash hazards while changing out and neutralizing liquid chemicals to the tissue analyzer, safety glasses and face shields were not being worn. 29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: Histology Utility Room: On or about 4/7/2016, a re-locatable power tap was being used in lieu of fixed wiring to provide electrical power to a 115 volt, 9 amp Tissue Analyzer. CORRECTED DURING THE INSPECTION Notice Citation #001, Item 001, 29 CFR 1910.305(g)(1)(iv)(A): as required by 29 CFR 1960.8(b): The employer failed to prohibit the permanent use of extension cords being uses to power two Air Master fans, a Coolit AC unit and a Skat abrasive blaster unit. The employer was cited for a violation of the same standard on 09/29/2014, Inspection #985783, 11/06/2014 of final order/settlement, and was abated by unplugging all of the extension cords at the end of each day at the workplace located at 510 Butler Avenue, Martinsburg, WV 25405.
