29 CFR 1926.451(e)(1):  When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as used as a means of access.  On or about April 5, 2016, an employee was exposed to a fall hazard greater than 6 feet to the ground level below, while applying metal lath to the exterior wall of a residential construction from a scaffold without a ladder or any other suitable form of access.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system or any other alternative fall protection measure.  The employer could not demonstrate that it was infeasible or created a greater hazard to use these systems, and had not developed an alternative plan:  On or about April 5, 2016, an employee was exposed to a fall hazard of greater than 6 feet to the ground level below, while applying metal lath to the exterior walls of a residential construction, without the use of fall protection equipment or an alternative fall protection plan.
