Employees performed set-up on a portable saw mill, multihead resaw, and other machines were exposed to mechanical, pneumatic, hydraulic, kinetic, etc. energy sources. b) On or about April 4, 2016, employees were exposed to caught-in, caught-by, or struck by hazards when the employer required the employees to operate machinery without proper pulley guarding. c) On or about April 4, 2016, employees were exposed to caught-in, caught-by, or struck by hazards when the employer required the employees to operate machinery without proper sprocket wheel and chain guarding.
