29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:   a) On or about April 4, 2016, the employer did not establish an energy control program to ensure employees who performed service and/or maintenance including set-up on machines or equipment were protected from unexpected energizing, start up or release of stored energy. Employees performed set-up on a portable saw mill, multihead resaw, and other machines were exposed to mechanical, pneumatic, hydraulic, kinetic, etc. energy sources.    b) On or about April 4, 2016, employees were exposed to caught-in, caught-by, or struck by hazards when the employer required the employees to operate machinery without proper pulley guarding.  c) On or about April 4, 2016, employees were exposed to caught-in, caught-by, or struck by hazards when the employer required the employees to operate machinery without proper sprocket wheel and chain guarding.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    On or about April 4, 2016, the employer did not establish an energy control program to ensure employees who performed service and/or maintenance including set-up on machines or equipment were protected from unexpected energizing, start up or release of stored energy. An employee was observed operating a portable saw mill cut off saw without proper pulley guards.  b. An employee was observed operating a portable planer without proper pulley guards.   c. An employee was observed operating a portable resaw without proper pulley guards.
