Yard Area, Debarker:  On or about 04/04/16, the debarker operator was exposed to a 10" wide hole beside the operator cab with a potential fall of 42" to the next level of the platform.    b.)  Sawmill, Edger/Liveroll Area:  On or about 04/04/16, employees were exposed to a 10"x15" hole in the walk platform between the live roll and edger, creating a tripping hazard.
