29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about March 30, 2016, at the above addressed job site, employees were exposed to struck-by eye injuries while installing metal lath with pneumatic staple guns and using adhesive without a means of eye protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about March 30, 2016, at the above addressed job site, employees were exposed to a fall hazard of approximately 9 feet while using an extension ladder that was not extended 3 feet above the upper roof landing and lacked a grasping device.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder:  On or about March 30, 2016, at the above addressed job site, employees were exposed to a fall hazard of about 9 feet while using an extension ladder that did not conform with the 1:4 horizontal-to-vertical ratio.

On or about March 30, 2016, at the above addressed job site, employees who had not been provided training in the use of ladders were exposed to a fall hazard of about 9 feet while working from an extension ladder.
