29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: Fourth and Fifth Floor Psychiatric Units: Protective puncture resistant gloves were not provided for nurses, mental health care workers, and any staff who were required to go through patients belongings where sharps such as knives and needles have been found.

29 CFR 1910.1030(d)(4)(iii)(A)(1): Contaminated sharps were not immediately discarded in containers that were closable, puncture resistant, leakproof and labeled or color-coded according to paragraph g(1)(i) of this standard. Fourth and Fifth Floor Psychiatric Units: During searches, portable sharp containers were not available rooms to provide for immediate sharps disposal where contaminated needles are found in patient belongings.
