29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On March 29, 2016 located on the south end of department 477/Z2, there was no frequent and regular inspections performed by a competent person to ensure the job-site, materials and equipment such as barricade systems and/or bracing devices were maintained and utilized prior to demolition work of the 1/4 Ton Free Standing Base Plate Jib Crane. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On March 29, 2016 located on the south end of department 477/Z2, employees were performing demolition operations on a 1/4 Ton Free Standing Base Plate Jib Crane had not been instructed on the recognition and avoidance of unsafe demolition procedures of the 1/4 Ton Jib Crane to eliminate or control any hazards associated with this operation while exposing employees to a struck-by hazard. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
