29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On April 4, 2016 at the 331 Main St., Omro jobsite; employees of Custom Metal Roofing were utilizing an extension ladder to access the roof above the garage. The extension ladder did not extend at least three feet past the landing.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a) On April 4, 2016 at the 331 Main St., Omro jobsite; employees of Custom Metal Roofing were installing metal roofing components on a stick framed building without fall protection. Custom Metal Roofing was previously cited for a violation of this Occupational Safety and Health Standard [29 CFR 1926.501(b)(13)], which was contained in osha inspection number 315288431, citation number 2, item number 1 and was affirmed as a final order on September 20, 2012, with respect to a workplace located at the Justice Center, Lac du Flambeau, Wisconsin 54538. Custom Metal Roofing was previously cited for a violation of this Occupational Safety and Health Standard [29 CFR 1926.501(b)(13)], which was contained in osha inspection number 307041319, citation number 2, item number 1 and was affirmed as a final order on September 21, 2006, with respect to a workplace located at 3705 N. Popko Circle, Mercer, Wisconsin 54547.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired. (a) On April 4, 2016 at the 331 Main St., Omro jobsite; employees of Custom Metal Roofing were utilizing an extension ladder which had damaged rails on the left side. Employees utilized the ladder to access the residential garage roof.
