29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:   On May 20, 2016, Juan Villaneva was issued citations and the employer did not verify, through documents that each citation was abated.  Abatement documentation was not received for the following violations:              a.  Citation 1 Item 1 and,         b.  Citation 2 Item 1a & b and,         c.  Citation 2 Item 2.   The Juan Villaneva was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1903.19(d)(1), which was contained in OSHA inspection number 1111282, citation number 2, item number 2 and was affirmed as a final order on 50/16/2016, with respect to a workplace located at  206 Greenview Dr., East Peoria, IL.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.     On or about March 30, 2016, employees were exposed to a fall hazard of approximately 17ft. 6in. on a construction site in Peoria, IL and the employer did not ensure the use of fall protection.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)    On or about March 30, 2016 employees were exposed to a struck-by hazard in the eye while using a pneumatic nail gun, on a construction site in Peoria, IL and the employer did not ensure a competent person inspected the job site for unsafe activity and took necessary steps to eliminate or control the hazards.    Juan Villaneva was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(2), which was contained in OSHA inspection number 1111282 citation number 1, item number 1B and was affirmed as a final order on March 28, 2016, with respect to a workplace 206 Greenview Dr., East Peoria, IL 61611.
