29 CFR 1926.502(h)(1)(iii): The safety monitor shall be on the same walking/working surface and within visual sighting distance of the employee being monitored; (a) Turner Morris Incorporated at 401 Main St., Longmont, CO 80501: On or before April 6th, 2016 the employer did not ensure that all employees were protected from fall hazards while conducting roofing operations. An employee was observed sweeping and traversing the roof eyebrow without a safety monitor in place from one end of the roof line to the other. This condition exposed the employee to a fall hazard of approximately 12 feet to the concrete sidewalk below.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: (a) Turner Morris Incorporated at 401 Main St., Longmont, CO 80501: On or before April 6th, 2016 the employer did not ensure that all employees were protected from fall hazards while conducting roofing operations. An employee was observed performing roofing work with an improperly adjusted rope grab which put excessive slack in the fall protection system. This condition exposed the employee to a fall hazard of approximately 12 feet to the concrete sidewalk below.
