29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent.   On or about April 5, 2016, at the Accounts Management Division, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2016:  a. On or about March 17, 2016, an employee incident report (OWCP-CA20) was not recorded for a respiratory condition which resulted from old carpets removal and replacement renovation project.  Case resulted in 1 day away from work.  b. On or about February 29, 2016, an employee incident report (OWCP-CA1) was not recorded for a respiratory condition which resulted from old carpets removal and replacement renovation project.  Case resulted in 143 days away from work.  NOTE: Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.

The agency's OSHA 300 logs and 300A summaries for calendar years 2016-2013 were not provided to the authorized government representative within four business hours.     NOTE: Certification of abatement is not required for this citation item since it was submitted prior to citation issuance.
