29 CFR 1910.22(a)(3): Every floor, working area, or passageway was not kept free from protruding nails, splinters, holes, or loose boards: Employees engaged in production activities were exposed to slip and trip hazards in that sulfuric acid had corroded the floor and caused the floor to heave. The heaving floor caused the bricks to come loose and resulted in holes in the following locations: a) C-Line Finishing area, crack floor. b) C-Line Charging area, crack floor. c) Travel path area and around work-stations throughout the Formation plant, crack floor. d) Acid Fill area, hole on the floor beside bricks and dyke section. e) Charging/Holding area, hole on the floor in front of float line 4-6. 29 CFR 1903.19(c)(1): Requires certification that the abatement of the above violation is complete.

29 CFR 1910.305(a)(2)(i): Temporary electrical power and lighting installations of 600 volts, nominal, or less were used for purposes other than those permitted in subparagraphs (a), (b) and (c) of this paragraph: Employees engaged in production activities on the C-Line area were exposed to electrical shock and fire hazards in that 120 volt AC power strip was used with an extension cord to power the MDT - 50 Air Checker machine, the IPad, and the scanner for inventory/production control. 29 CFR 1903.19(c)(1): Requires certification that the abatement of the above violation is complete.

29 CFR 1910.305(g)(1)(iii): Flexible cords and cables were used for purposes prohibited by subparagraphs (a) through (e) of this paragraph: a) Employees engaged in production activities on the C-Line area were exposed to electrical shock and fire hazards in that an extension cord was used as a substitute for the fixed wiring of a structure to supplying power to the IPad/Scanner for inventory/production control. b) Employees engaged in production activities on the C-Line area were exposed to electrical shock and fire hazards in that an extension cord was used as a substitute for the fixed wiring of a structure to supplying power to the power strip which in turn energizes the MDT - 50 Air Checker machine, the IPad, and the scanner for inventory/production control. 29 CFR 1903.19(c)(1): Requires certification that the abatement of the above violation is complete.
