On or about April 6, 2016, employees using nail guns were not wearing safety glasses and were being exposed to eye injuries. A. J. Wells Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1) which was contained in OSHA inspection number 892311, citation number 01, item number 001 and was affirmed as a final order on April 2, 2013, with respect to a workplace located at 13226 Joeandy Road North, Jacksonville, FL 32225.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. on or about April 6, 2016, four employees installing architectural shingles on a 5:12 pitch roof were not protected from falling 10 feet by the use of a guardrail system, safety net system, personal fall arrest system or any other type of fall protection method. A.J. Wells Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 892311, citation number 02, item number 001 and was affirmed as a final order on April 2, 2013, with respect to a workplace located at 13226 Joeandy Road North, Jacksonville, FL 32225.
