29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about April 5, 2016, at the subject job site - Employees working below residential roofing operations were not wearing hard hats to protect against falling material and debris. Employees were exposed to struck-by hazards.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    On or about April 5, 2016, at the subject job site - Employees using pneumatic nail guns did not wear eye protection (safety glasses), thereby exposing employees to eye injuries from flying debris and broken nails.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about April 5, 2016, at the subject worksite - Three ladders were used to gain access to the roof of the house that did not extend at least three (3) feet above the landing.  Employees were exposed to falling approximately 10 feet to 13 feet to the ground below while mounting and dismounting the ladder.    No abatement information is required for this item.
