29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a. On or about March 29, 2016, at the fire pump electrical switchgear cabinet, within the Main Electrical Room of the Central Energy Plant building, adjacent to a new construction hospital building, two (2) subcontracted employees were permitted to work in proximity to energized 480 volt, 3 Phase, 400 ampere, electrical power circuits and exposed to electrical shock hazards and burns resulting from an electrical arc flash. b. On or about March 29, 2016, at the fire pump electrical switchgear cabinet, within the Main Electrical Room of the Central Energy Plant building, adjacent to a new construction hospital building, an employee and a subcontracted employee were permitted to work in proximity to energized 480 volt, 3 Phase, 400 ampere, electrical power circuits, which resulted in burns from an electrical arc flash that injured both employees.
